PATIENT INFORMATION

 Lakeview Obgyn is open Monday through Friday by appointment only. When calling to schedule an appointment, please state the purpose of the appointment clearly to allow for the appropriate amount of time to be scheduled. 

We require 24 hours notice to cancel your appointment. Please be considerate of other patients who are waiting for appointments. Last minute cancellations are subject to a cancellation charge that will be charged to your account. Just remember to call and this charge will be waived.

The office and phone lines are open from 8:30 am on weekdays, and our reception staff handles all of our calls. If you need to speak directly to a doctor, one will return your call when time permits. Please be patient as our doctor is very busy during office hours. All calls will be attempted to be answered within 24 hours. The doctor is not interrupted during patient hours to answer a phone call unless it is an emergency. You wouldn't want your office visit to be interrupted by a phone call, so please extend that courtesy to others.

In order to provide you with the best possible care, we need the most up-to-date information about you.  If you are a new patient or an established patient who has not visited our office within the past three months, we will need an updated registration form at your next appointment.

When you come in for your appointment, you may be asked to sign several forms, including other HIPAA required forms and other office policy forms authorizing us to submit billing information to your insurance company. 

Please be sure to bring these to your first appointment:

• Insurance card and Identification
• Physician referral form (if required by your insurance)
• List of all medications prescription and over-the-counter drugs and herbs/vitamins with dosages that you are taking
• Recent test results
